Preparing for your Visit:

Booking Your Appointment:  Please let the receptionist know what type of visit you need. They will book the appropriate appointment time. Please indicate if you need a regular visit, a longer visit for counselling, a complete physical examination or simply an injection. 

 

In addition to appointments that are scheduled in advance, we leave appointment slots available daily for urgent matters.  In this way, we hope to accommodate any unexpected needs you may have.

 

 

Parking: There is free parking for our building. 

 

 

What to Bring:You must show your OHIP card at every visit. Bring all your current medications, including drug dosages to your appointments.

PLEASE NOTE: PATIENTS PRESENTING WITH AN EXPIRED/INVALID OHIP HEALTH CARD WILL BE REQUIRED TO PAY $60 DEPOSIT FOR YOUR VISIT ($63 if VISA/DEBIT). This will be refunded minus $3 processing fee once OHIP pays for your bill if you update us with your Health Card Version Code within 30 days of your visit. If you do not have a valid OHIP card, you may be invoiced directly for your visit according to non-OHIP fees.
